Scheduling has moved to our new electronic health record platform. Both new and returning clients can expect to receive New Client Paperwork as we transition to Optimantra. 

A few reminders about paperwork and forms...

Please answer questions as completely as possible. Your responses helps us provide safe, timely care, and can reduce the length of time needed to develop your plan of care.

Forms completed on behalf of someone else are only accepted from legal guardians of minors age 14 and younger.
Your insurance information is needed to determine if we are an in network provider, and for prescriptions/ prior authorizations. 

A word for our new or prospective clients

Dynamic Minds currently works with two billing partners, Alma and Headway. Our staff will notify you which billing partner you need to sign up with once we have verified your insurance. 

Online appointments will now be through your new patient portal - simply click on the link in your confirmation email or text.
